What if I do not have insurance?

Payment is due at the time of service. These rates include diagnosis and procedure codes that may allow you to seek reimbursement from your insurance. Receipts are available upon request 24-48 hours after your session.

How much does Physical Therapy Cost with insurance?

The cost of care depends on your individual insurance plan. Most insurance plans provide some level of physical therapy coverage.

Some patients have a copay for each visit. Others, even when seen in-network, may be responsible for approximately $145-$200 per visit until their deductible has been met.

We recommend calling your insurance company before your first visit so you have a clear understanding of your financial responsibility.

What if I was in a car accident?

We can typically treat motor vehicle accident cases when you have available PIP (Personal Injury Protection) coverage, and we bill that coverage on your behalf.

At this time, we do not accept:

Third-party payer cases
Cases in litigation
Exhausted funds
Letters of lien

If you were a passenger, we can only bill your own available coverage, not the driver's insurance.

Do I need a referral?

Washington is a direct access state, which means you may be seen by a physical therapist without a physician referral. However, your insurance may still require a referral or prior authorization in order to reimburse care.

A few important exceptions include:

MVA & LNI patients must have a referral from an MD for physical therapy

Some insurance plans may also require approval through third-party utilization management companies such as eviCore/careCore

Our billing team can help determine whether a referral is needed, but we also recommend confirming requirements directly with your insurance company.

What is eviCore / careCore, and how does it affect my appointments?

eviCore and careCore are names often used for the same type of third-party company hired by insurance carriers to review medical records and determine whether ongoing care is approved.

If your insurance uses this process, you may receive letters in the mail regarding approval or denial of visits. In many cases, patients are initially approved for at least 6 visits, and our team will request additional visits as needed. If more visits are not approved, we will discuss your options with you directly.

What is your cancellation policy?

If you need to cancel or reschedule, please contact our front desk as soon as possible. If you are unable to reach someone, you may leave a voicemail.

Cancellations or rescheduling within 24 hours of your scheduled appointment incur a $150 fee that is not billable to insurance.

Does Innova charge a processing fee for credit card transactions?

Yes. Innova charges a 3% processing fee for credit card transactions.
