At Innova Physical Therapy, we believe the thorax is one of the most important — and most misunderstood — regions in the body.

Many clinics reduce this area to the thoracic spine alone. At Innova, we take a far more advanced view. We assess and treat the thorax as a three-dimensional system made up of the rib cage, thoracic spine, sternum, diaphragm, surrounding musculature, pressure systems, and its relationship to the neck, shoulders, abdomen, pelvis, and nervous system.

This matters because the thorax does far more than simply “sit in the middle of the body.” It plays a central role in breathing, posture, balance, force transfer, core control, upper extremity function, pressure regulation, and the body's ability to move with both efficiency and resilience.

The thorax is the central chamber of the body. It is where multiple critical systems intersect and influence one another.

A healthy thorax contributes to:

We Treat The Thorax Differently

We use a 3D approach to the thorax, recognizing that the rib cage must expand, recoil, rotate, transfer load, regulate pressure, and coordinate with the diaphragm, abdominal system, spine, and extremities. This broader perspective allows us to identify barriers that are often missed when treatment is limited to spinal stiffness or posture alone.

Our approach considers the thorax in relation to:

Respiration

Breathing is not only about oxygen exchange. It also shapes posture, rib mechanics, pressure control, trunk muscle function, and nervous system state. Poor thoracic expansion or altered breathing patterns can contribute to pain, tension, fatigue, poor core engagement, and compensation throughout the body.

Center of mass over base of support

The thorax plays a major role in how the body organizes itself over the feet and responds to gravity. If the thorax is poorly positioned or poorly controlled, balance strategies often become inefficient, creating excess strain elsewhere.

Sympathetic drive

The thorax is deeply tied to the body's stress response. Restriction, over-bracing, breath holding, and chronic tension can all reinforce a state of sympathetic dominance, which may amplify pain, guarding, fatigue, and difficulty recovering.

Trunk mobility

Rotation, side bending, flexion, extension, and rib movement all contribute to efficient motion. If the thorax cannot move well, the neck, low back, shoulders, and hips often compensate.

Transmission of loads

The thorax helps distribute force between the upper body, trunk, and lower body. When it is not functioning efficiently, load transfer becomes less effective, often leading to overwork, instability, or pain elsewhere.

Thoracic, cranial, and abdominal pressure regulation

The body relies on pressure management for movement, support, and control. The thorax is central to these relationships. Disruption in one region may influence the others, affecting performance, stability, breathing, and symptom patterns.

Neurovascular supply to the upper extremity

The thorax also influences the mechanical environment of the nerves and blood vessels serving the arm. Poor thoracic position, rib mechanics, or surrounding tissue tension may contribute to upper extremity symptoms, especially when the problem is not just local to the arm or shoulder.

Core control and strength

True core function is not just about abdominal tightening. It depends on the thorax, diaphragm, abdominal wall, spine, and pelvis working together. A thorax that is stiff, collapsed, flared, or poorly coordinated can compromise the foundation for strength and control.
