Understanding your insurance benefits is important and knowing your benefit for physical therapy will help ensure a positive experience. Most insurance plans cover physical therapy at some level. We encourage you to call your insurance company and check on your physical therapy benefit before you come in. As a courtesy to you, we can call to check on your benefits for you. If your plan requires a referral from your Primary Care Physician, please obtain it before your scheduled appointment.

The appointments made for you represent a time set aside specifically for you and your therapist. We value your time and ask that you value ours by giving at least 24 hours notice for any cancellations or changes to your appointment.

Your First Appointment

  • Bring a copay if applicable.
  • Bring a prescription or referral from your physician.
  • Bring all related medical records and/or diagnostic reports (X-rays, MRI, etc).
  • Bring a valid insurance card or claim information (motor vehicle or Labor & Industries).
  • Please print the new patient forms, fill them out, and bring them with you to your appointment.
  • Bring comfortable clothing that does not restrict your motion and will facilitate the visualization of the spine.
  • Dancers: Please bring tights, a leotard and some shorts, as well as pointe shoes (if applicable) and your dance bag.
  • Performing Artists: Please bring all relevant equipment such as musical instruments.
  • Orthotic appointments: Be sure to bring the footwear that we are crafting the orthotic for, any prior crafted orthotic devices and any braces or splints that you may be using.
  • TMD appointments: Please bring all splints, retainers, night guards, and a copy of your panoramic x-ray.
